The Versatility of the Anterolateral Thigh Perforator Flap for Reconstruction of Soft Tissue Defect of the Upper Extremity

Abstract
Purpose: This study investigated the versatility of anterolateral thigh (ALT) perforator flap in reconstruction of soft tissue defect of upper extremity and hand.

Materials and Methods: There were forty-three patients who had been taken soft tissue reconstruction with ALT perforator flap. There were 27 males and 16 females between 7 and 58 years (mean, 37 years). Type of used flap were as follow: cutaneous flap (38 cases), adipofasciocutaneous flap (2 cases) and musculocutaneous flap (3 cases). Flap size averaged 149.6 cm2 (21~450 cm2). 36 cases were musculocutaneous perforator and 7 were septocutaneous perforator. T-shaped pedicles were used to reconstruct and to preserve major artery of upper extremity in 5 cases.

Results: The overall flap success rate was 99.8%. There were five complication related flap. There were two cases of venous congestion that required decompression of pedicle and reanastomosis after exploration. There were three cases of marginal necrosis of flap that were treated by debridement and skin closure. Although three cases had donor site breakdown, it had healed by secondary intension.

Conclusion: An anterolateral thigh flap is a versatile and useful flap for a variety of upper extremity soft tissue reconstruction.
